Review Morich Hockey waiver

This Participant Waiver, Assumption of Risk, Release of Liability, Medical Authorization, and Training Video Consent applies to participation in any Morich Hockey LLC activity, including on-ice hockey training, skating instruction, skills clinics, private lessons, small-group lessons, shooting sessions, off-ice training, dryland training, strength and conditioning, speed and agility work, mobility work, camps, clinics, evaluations, warmups, cooldowns, use of equipment, and any related activity.

For purposes of this agreement, “Morich Hockey” means Morich Hockey LLC, Brett Morich, its owners, coaches, instructors, trainers, employees, contractors, volunteers, representatives, agents, successors, assigns, insurers, facility owners, rink operators, gym operators, property owners, sponsors, and any other parties involved in hosting, operating, or supporting the activity.

1. Participant and Booking Information

This agreement applies to the participant, parent/guardian, activity, session, booking, payment, emergency contact, and medical information submitted through the Morich Hockey website, checkout, registration, account, inquiry, or booking process.

By accepting this agreement electronically, I confirm that the information I submitted is true and accurate to the best of my knowledge.

If the participant is under 18, I confirm that I am the participant’s parent or legal guardian and that I have authority to register the participant for Morich Hockey activities.

2. Acknowledgment of Hockey and Training Risks

I understand that hockey, skating, off-ice training, and athletic conditioning involve inherent and unavoidable risks. These risks exist even when activities are supervised, rules are followed, and reasonable safety measures are used.

These risks include, but are not limited to:

  • Falls on ice, turf, flooring, pavement, or other surfaces
  • Collisions with boards, glass, nets, sticks, pucks, training equipment, other participants, coaches, spectators, or facility structures
  • Contact with skates, sticks, pucks, weights, bands, shooting equipment, balls, cones, hurdles, ladders, sleds, or other training equipment
  • Cuts, bruises, sprains, strains, fractures, dental injuries, eye injuries, facial injuries, concussions, head injuries, neck injuries, back injuries, spinal injuries, heat-related illness, dehydration, overexertion, cardiac events, illness, permanent disability, paralysis, or death
  • Risks related to strength training, jumping, sprinting, agility drills, shooting drills, skating drills, balance work, conditioning, and physical exertion
  • Risks caused by the actions, inactions, mistakes, or negligence of other participants, parents, spectators, facility users, or third parties
  • Risks related to ice conditions, floor conditions, facility conditions, equipment condition, weather, crowding, and limited visibility
  • Risks related to the participant’s own physical condition, effort level, decision-making, or failure to follow instructions

I understand that no coach, trainer, program, drill, facility, or piece of equipment can eliminate all risk of injury.

3. Fitness to Participate

I confirm that the participant is physically and medically able to participate in the selected activity.

I understand that Morich Hockey does not provide medical advice, medical screening, diagnosis, physical therapy, rehabilitation, or medical treatment.

I agree that the participant should not participate if the participant is injured, ill, experiencing symptoms, under medical restriction, or otherwise unable to safely participate. I agree to disclose relevant medical limitations, allergies, injuries, medications, asthma, heart conditions, concussion history, or other conditions that may affect safe participation.

4. Equipment and Safety Rules

I understand that the participant is responsible for using proper, well-fitting, and safe equipment for the activity.

For on-ice activities, required equipment may include helmet, skates, gloves, stick, protective pads, mouthguard, neck protection where applicable, and any other equipment required by Morich Hockey, the facility, the participant’s team, or applicable rules.

For off-ice activities, required equipment may include athletic shoes, appropriate clothing, hydration, and any other equipment requested by Morich Hockey.

The participant agrees to follow all coach instructions, posted facility rules, safety rules, and behavior expectations. Morich Hockey may remove any participant from an activity, without refund, if the participant acts unsafely, refuses to follow instructions, disrupts the session, bullies others, uses inappropriate language, damages property, or creates an unreasonable risk to themselves or others.

5. Assumption of Risk

I knowingly and voluntarily accept and assume all risks associated with the participant’s participation in Morich Hockey activities, whether known or unknown, anticipated or unanticipated, inherent or otherwise.

If I am an adult participant, I accept these risks for myself.

If I am signing as a parent or legal guardian, I understand and acknowledge these risks on behalf of myself and give permission for the minor participant to participate with full knowledge of these risks.

6. Release of Liability

To the fullest extent permitted by Michigan law, I release, waive, and discharge Morich Hockey from any and all claims, demands, causes of action, damages, losses, costs, or expenses arising out of or related to participation in Morich Hockey activities, including claims related to personal injury, property damage, illness, disability, or death.

This release applies to claims arising from ordinary negligence to the fullest extent permitted by law. This release does not apply to claims that cannot legally be released under applicable law, including claims involving gross negligence, willful misconduct, or intentional misconduct where such claims cannot be released.

If I am signing as a parent or legal guardian of a minor participant, I understand that this agreement is intended to be enforced to the fullest extent permitted by Michigan law. I also understand that some legal rights belonging to a minor may not be waivable by a parent or guardian under Michigan law. Even so, I agree that I am releasing my own claims, including any claims I personally may have for loss of services, emotional distress, medical expenses I personally incur, reimbursement, or other damages arising from the participant’s involvement, to the fullest extent permitted by law.

7. Medical Authorization

In the event of an injury, illness, or emergency, I authorize Morich Hockey, its coaches, trainers, staff, volunteers, facility staff, or other responsible adults to obtain or arrange emergency medical care for the participant if I cannot be reached or if immediate care appears necessary.

I understand that Morich Hockey is not responsible for the cost of medical care, ambulance transportation, emergency treatment, follow-up care, rehabilitation, or related expenses. I accept responsibility for all such costs.

8. Insurance

I understand that Morich Hockey does not provide health, medical, disability, or accident insurance for participants. I confirm that the participant is covered by appropriate health insurance or that I accept financial responsibility for any medical costs related to participation.

9. Facility and Third-Party Rules

I understand that Morich Hockey activities may take place at third-party ice rinks, gyms, training facilities, fields, parking lots, or other locations. I agree that the participant and any accompanying parent, guardian, sibling, or spectator must follow all facility rules and instructions.

I understand that third-party facilities may require separate waivers, rules, payments, or safety procedures.

10. Training Photos and Video

I understand and agree that Morich Hockey may photograph or record the participant during Morich Hockey activities for coaching, instruction, player development, safety, internal review, and communication with the participant or the participant’s parent/guardian.

These recordings may include videos of skating, shooting, skills work, off-ice drills, strength and conditioning, or other training activities.

Morich Hockey will not sell the participant’s image or use the participant’s name, photo, or video for paid advertising without additional permission.

Promotional use of photos or videos for Morich Hockey’s website, social media, marketing, or advertising will require separate optional consent.

11. Payments, Cancellations, and Refunds

I understand that payment, cancellation, credit, and refund policies are determined by Morich Hockey and may vary by activity. I agree to review and follow the applicable policy listed at checkout, on the schedule page, or in the activity description.

12. Electronic Signature and Consent to Electronic Records

By checking the required waiver box, typing my name, and submitting registration or payment through the Morich Hockey website, I agree that my electronic acceptance has the same legal effect as a handwritten signature.

I agree to receive, access, and sign this waiver electronically. I understand that I may request a copy of this waiver from Morich Hockey.

I confirm that:

  • I have had access to the full waiver before submitting payment or registration.
  • I have read this agreement carefully.
  • I understand that hockey and athletic training involve serious risks, including injury, permanent disability, or death.
  • If the participant is under 18, I am the participant’s parent or legal guardian and have authority to register the participant.
  • I understand that I am accepting this agreement on behalf of myself and, to the fullest extent permitted by law, on behalf of the participant.
  • I understand that some claims involving minors may not be waivable under Michigan law, but I am releasing my own claims and accepting all terms enforceable against me to the fullest extent permitted by law.
  • I understand that this agreement applies to the selected Morich Hockey activity and related participation.
  • I agree voluntarily.

Morich Hockey may store a record of my acceptance, including my typed name, relationship to the participant, waiver version, date and time of acceptance, participant name, session or activity, IP address, browser/device information, and related booking information.

13. Severability

If any part of this agreement is found to be invalid or unenforceable, the remaining parts will remain in effect to the fullest extent permitted by law.

14. Governing Law

This agreement is governed by the laws of the State of Michigan, without regard to conflict-of-law rules.

Review refund policy

Camp registrations are refundable up to 48 hours before the first scheduled camp session. Cancellations requested within 48 hours of the first camp session are not guaranteed a refund, but Morich Hockey may review special circumstances on a case-by-case basis. If Morich Hockey cancels the camp or a session cannot be held due to facility issues, weather, or other circumstances outside normal operations, families will receive either a makeup option, credit, or refund depending on the situation. Refunds are returned to the original payment method when possible. Stripe/payment processing fees may be non-refundable.
